A Pharmaceutical Opinion is given each time a product is dispensed. In addition it is:

in a standardised format when there is a complex alert, a doubt whether or not resolved, a need for surveillance, an original intervention, a known vulnerability, pathogenic behaviour, specific precautions for use, a particular substitution or a need for internal information in the pharmacy,

communicated when legally required (in the case of refusal or modification) or if the pharmacist judges it useful to inform the prescribing physician of precise information he may require. Formal communication is a condition for legal protection of interest . |
When simple payment for a product is the only written trace of the delivery, it is implicitly but necessarily the result of a conclusive analysis in the eyes of the law.
The Pharmaceutical Opinion is an instrument of therapeutic cooperation that clarifies responsibilities, demonstrates and enhances know-how and communication between physicians and pharmacists.
How does the Pharmaceutical Opinion work ?
The Pharmaceutical Opinion must explain the analyses, reasons and decisions of the pharmacist and presupposes : |

a rigorous methodology for gathering information, |
|
| |
a faithful and understandable transcription of this information, |
|
| |
a standardised format.

The Pharmaceutical Opinion is a document that can be established manually or with a computer and that contains:
|
|
administrative identification (of the pharmacist, the patient, the person dispensing, the prescribing physician (if applicable)), |
|
| |
the patient profile (pathologies, personal and family medical history, biometrics data, relevant laboratory analysis results, and any relevant psychosocial data), |
|
| |
the nature of the problem and the pharmacist's arguments, if possible along with a proposal for replacement, |
|
| |
the origin of the information justifying the pharmacist's intervention, |
|
| |
the possible reminder of the medical basis which motivated the prescription, |
|
| |
the decision taken by the pharmacist,
|
|
| |
specific information given to the patient, |
|
| |
the date and signature (validation). |
